Guardianship Referrals

To refer someone for guardianship please click to download and complete the following file, then submit to Sarah Walker via email at sarah@thesolutionscenter.org for the following Indiana counties: Crawford, Dubois, Orange or Washington. 
Submit to Tana Hutcherson via email at tana@thesolutionscenter.org for the Indiana counties of Lawrence, Monroe & Owen.

  • To be eligible for referral to SIAGS, an individual must meet the following criteria:

  • Be at least 18 years of age

  • Reside in a 24-hour care setting, such as a nursing home or group home

  • Live in one of the following Indiana counties: Lawrence, Orange, Washington, Crawford, Dubois, Monroe, or Owen

  • Meet the legal definition of incapacity under Indiana law

  • Have no willing or able family member available to serve as guardian

  • Provide a completed Physician’s Statement of Incapacity signed by a licensed physician

Types of Guardianships

Guardian of Person

A guardian of the person is a court-appointed individual responsible for the daily care, health, safety, and personal needs of an incapacitated adult or minor (the ward). They make critical decisions regarding housing, medical treatment, education, and clothing, ensuring the ward's physical and emotional well-being.

Guardian of Estate

Adult Protective Services (APS)

The Adult Protective Services program was established to receive and investigate reports regarding adults within the state of Indiana who may be endangered and, as appropriate, to coordinate a proper response to protect endangered adults who are victims of abuse, neglect, or exploitation.
